Click the "Download" button for each form.

If the form downloads without opening in a new tab, open the download in the build in PDF software on your computer.

For Mac users, it is the "Preview" app.

For PC users, it is "Microsoft Edge".

​Select the text icon from the tool bar and complete form. 

You may also print the forms, complete in pen, take a photo of the completed forms and upload the photo using the button above.

If the form opens in a new tab, select the text icon from the tool bar and complete form. 

Save file and upload by clicking the "Upload Completed Forms" button above.

Please save file with the dancer's name in the file name.

If you are having difficulties opening the PDF, you can use Canva and upload the PDF there. Canva will let you edit and export the PDF for free.
